Scene Assessment

A 50yo construction worker fell 15 feet from scaffolding, striking his left chest on a steel beam. He is fully conscious but in agonizing pain. He is taking very shallow, rapid breaths. You observe a large segment of his left lateral chest wall moving inwards during inspiration and outwards during expiration (paradoxical movement).
