Assessment

You arrive at a high-speed T-bone MVA. The driver is extricated but unconscious. His right chest is bruised and completely silent on auscultation. When you percuss his right chest, it sounds dull and "thud-like" (unlike the hyper-resonance of a pneumothorax). His neck veins are flat. He is in profound shock.
